Reporting Guidance

As per the Notice of the Award letters for Year 5 of Funding Opportunity Announcement PS13-1308, four reports are due during the twelve month budget period:

Annual Performance Report

Reporting information for PS13-1308: Promoting Adolescent Health Through School-Based HIV/STD Prevention and School-Based Surveillance will be updated in January 2018.  For your convenience, instructions and optional templates are provided below.
