As per the Notice of the Award letters for Year 5 of Funding Opportunity Announcement PS13-1308, four reports are due during the twelve month budget period:
- Performance Measure Report #1 (Strategies 2 & 3 only) – due March 1, 2018 (via wwwn.cdc.gov/pers)
- Annual Performance Report – due April 2, 2018 (via www.grantsolutions.gov)
- Performance Measure Report #2 (Strategies 2 & 3 only) – due September 4, 2018 (via wwwn.cdc.gov/pers)
- Work Plan Status Report #2 – due August 31, 2018 (via www.grantsolutions.gov)
