Emerging Practices in Diabetes Series

State health departments are exploring and testing new and innovative approaches that will provide critical insight and lessons learned. We refer to these approaches as emerging practices. The goal of the Emerging Practices in Diabetes series is to summarize and share information on these proven practices to inform the work of other states, cities, or communities working in the same or related areas under 1305 and 1422.

This document examines the role of pharmacists in chronic disease management and highlights experiences from three 1305 grantees—Colorado, Iowa, and Ohio—in involving pharmacists in medication therapy management and team-based care. The states describe the roles they and their partners played, facilitating factors and barriers, initial results, and lessons learned.

This document provides an overview of a technical assistance tool developed by CDC staff to explain key drivers associated with engaging community health workers (CHWs) in diabetes self-management education (DSME) programs. It also highlights examples from three states that have done significant work in supporting the CHW workforce, including training, certification, and reimbursement initiatives. This report provides details on how three organizations built diabetes self-management education (DSME) programs using Stanford University’s Diabetes Self-Management Program (DSMP) curriculum and also met all of the National Standards necessary to be accredited by the American Association of Diabetes Educators or recognized by the American Diabetes Association.

Approaches to Employer Coverage of the National Diabetes Prevention Program for Employees at Risk for Type 2 Diabetes [PDF - 550 KB]
This document tells the stories of five employers offering the National DPP’s lifestyle change program as a covered health or wellness benefit for their employees and discusses the varied approaches, challenges, and key factors contributing to their success. A table summarizing and comparing the different approaches is provided at the end of the document. This information will be helpful for other employers interested in providing coverage for the National DPP as well as CDC grantees and other organizations that have a stake in scaling and sustaining the program by expanding access to employees nationwide.
