Diabetes Programs & Initiatives

CDC provides national leadership and support for the implementation of a high-impact prevention approach to reducing new diabetes cases by using combinations of scientifically proven, cost-effective, and scalable interventions and prevention strategies directed towards the most vulnerable populations in the US who are most affected by, or at greatest risk for diabetes. National Parnetship. Community Based. Scaling the National Diabetes Prevention Program in Underserved Areas (RFA-DP17-1705)
On April 11, 2017, CDC released a new five year Funding Opportunity Announcement seeking proposals from national and regional organizations to help scale the National DPP in Underserved Areas. Applications were due June 12, 2017.

CDC’s Division of Diabetes Translation (DDT) funds state and local health departments to support programs and activities to prevent or delay the onset of type 2 diabetes and to improve health outcomes for people diagnosed with diabetes.CDC’s funding and activities are part of two multi-program cooperative agreements that address type 2 diabetes; heart disease and stroke; nutrition, physical activity, and obesity; and school health.

This is a public-private partnership of community organizations, private insurers, health care organizations, employers, and government agencies. Partners work to establish local evidence-based lifestyle change programs for people at high risk for type 2 diabetes.

The National Diabetes Education Program (NDEP) works with partners to reduce the burden of diabetes and prediabetes by facilitating the adoption of proven approaches to prevent or delay the onset of type 2 diabetes and the complications of diabetes.

The Chronic Disease Initiative (CKD) is designed to provide comprehensive public health strategies for promoting kidney health. These strategies seek to prevent and control risk factors for CKD, to raise awareness, to promote early diagnosis, and to improve outcomes and quality of life for those living with CKD.

The mission of the Native Diabetes Wellness Program is to work with a growing circle of partners to address the health inequities so starkly revealed by the number of people with diabetes in Indian Country. With social justice and respect for Native and Western science as grounding principles, we strive to support community efforts to promote health and prevent type 2 diabetes.

The Vison Heath Initiative (VHI) promotes vision health and quality of life for all populations, through all life stages, by preventing and controlling eye diseases, eye injury, and vision loss resulting in disability. VHI, consists of a team in CDC’s Division of Diabetes Translation, collaborated with diverse stakeholders to develop a coordinated public health framework to promote the nation’s vision health across the lifespan.

Appalachian Diabetes Control and Translations
The goal of this project is to reduce the impact of diabetes on people who live in high-risk (distressed) counties in the Appalachian Region of the United States.

Vulnerable Populations
This program has been developed to reduce morbidity and premature mortality, and to eliminate health disparities associated with diabetes in many communities and regions across the country.

Prevent Diabetes STAT: Screen, Test, Act – Today™
With more than 86 million Americans living with prediabetes and nearly 90 percent of them unaware of it, the American Medical Association (AMA) and the Centers for Disease Control and Prevention (CDC) have joined forces to take urgent action to Prevent Diabetes STAT and are urging others to join in this critical effort.
