Health System Transformation

The US healthcare system is in a time of change like never before. The result is a unique opportunity for public health, health plans, hospitals, doctors and communities to work together so that we can keep people healthy and spend smarter. The American healthcare system is in the midst of unprecedented change. The U.S. health care and public health systems are both now positioned to place greater emphasis on better care, smarter spending, and healthier people.

Key elements emerging in this transformation include new structures for integrating and coordinating services, a renewed focus on patient engagement and patient-centered care, and new payment models based on the value of population-based health outcomes rather than the volume of services delivered. This period of change is creating important opportunities to establish effective, more sustainable models to improve population health.

To support this change, the CDC Office of the Associate Director for Policy has developed several resources and tools to help further catalyze health system transformation.
