Competitive Component

Current competitive component recipients are to provide a brief summary of work conducted thus far, including successes, challenges, and technical assistance needs. Recipients must use the Work Plan Template, Project Narrative Summary Section. Label the Work Plan file “Year-4 Work Plan-Competitive” and upload to Grantsolutions as an attachment under the “Miscellaneous” attachments section.

Is there a template for the Competitive Work plan and if so, do we get that from our project officer?

The template for the Competitive Component Work plan is the same as the Core Work plan. Send a request for a blank work plan template to oshmista@cdc.gov and cc your project officer.

Is there a separate template for the Competitive Budget and if so, do we get that from our project officer?

You may use the same template and allocate amounts to the competitive budget. If you wish to use a separate budget form, send a request for a blank budget template to oshmista@cdc.gov and cc your project officer, and allocate everything to the competitive columns.

Core Component

Update Year 2 APR (October 1, 2016 – March 29, 2017)

Use the pre-populated fields in the Excel template.

Update progress to activities and/or objectives that occurred since the last APR.

Year 3 APR – complete the first 6 months (March 30 – September 30, 2017)

Add progress to activities and objectives from March 30th – September 30, 2017.

Should we still submit our Excel documents to our Project Officer?

Yes, please send a copy of the Excel version of the templates to your Project Officer.
