PAsS: Program Enrollment

  1. Download and complete the application form:
    PAsS Enrollment Form [PDF – 371 KB]
  2. E-mail the completed form to ZYJ1@cdc.gov or fax it to (770) 488-4097. Notification of the enrollment usually occurs within 72 hours after the program receives the application.
  3. Enrolled laboratories receive quality-assurance materials every April and September.
