CDC’s Behavioral Risk Factor Surveillance Survey (BRFSS):
The BRFSS is an on-going telephone health survey that tracks health conditions and risk behaviors in all 50 states, the District of Columbia, Puerto Rico, the U.S Virgin Islands, and Guam.

Instructions: Choose the state. Choose the data year. Click on the data category to view the survey questions for which data are available. For example, under cardiovascular disease, data are available for heart attack, coronary heart disease, and stroke. For each question, the data can be grouped by gender, age, race, income, or education.

Contact the state department of health and human services or the local health department for statistics specific to study area.

CDC Wonder: Mortality Database: Underlying Cause of Death:
The Compressed Mortality database contains mortality and population counts for all U.S. counties for the years 1979 to 2007. You can get counts and rates of death by underlying cause of death; grouped by state, county, age, race, sex, and year.

Instructions: Go to the Web page; read the data use restrictions and agree to abide by data use terms before proceeding into the database. Select the desired grouping of results, state, age group, and demographic information. Indicate the cause of death to identify the nature of, injury intent and the injury mechanism. Click send to compile the data table.
