Notice of Award Fields

  • 3. Assistance Type
    Denotes cooperative agreement or grant.
  • 4. Grant Number
    Identifying number for the award.
  • 6. period of performance
    Defines the period of performance.
  • 7. Budget Period
    Defines the 12 month period that corresponds to the Total Approved Budget.
  • 9b. Recipient Project Director
    Project Director/Principal Investigator identified.
  • 10a. Recipient Authorizing Official
    Identifies the recipient Authorized Organizational Representative.
  • 11L. Total Approved Budget
    Designates the total amount that the recipient is authorized to expend for the budget period.
  • 11m. Federal Share
    Identifies CDC’s share of the total approved budget.
  • 11n. Non-Federal Share
    Identifies the recipient share of the total approved budget required through matching or cost sharing.
  • Grants Management Official
    CDC Grants Management Officer.
  • 18b and 19. EIN/DUNS
    Recipient Employer Identification Number/Dun & Bradstreet Number.
  • 21b. Document Number
    Identifies the account in the Payment Management System.
