This map shows the concentrations of counties with the highest prevalence for heart disease – meaning the top quintile – are located primarily in Louisiana, Alabama, Tennessee, Oklahoma, West Virginia, Ohio, Pennsylvania, and New York, with pockets located along the eastern seaboard, Michigan, Arkansas, Arkansas, Missouri, and Nebraska.
