Heart Disease Hospitalization Rates for 2010 through 2012 for Whites Aged 65 Years and Older by County. The map shows that concentrations of counties with the highest heart disease hospitalization rates - meaning the top quintile - are located primarily in Louisiana, Tennessee, Kentucky, West Virginia, Ohio, and Pennsylvania. Pockets of high-rate counties also were found in Alabama, Mississippi, Arkansas, Missouri, Virginia, Georgia, North Carolina, and South Carolina.
