Methods for State Oral Health Surveys that began data collection in 2010

Alaska

The survey was completed during the 2010-2011 school year. The sampling frame consisted of on-site elementary schools with a kindergarten and/or 3rd grade enrollment of 15 or more (correspondence and distance education schools were not included in the sampling frame). With implicit stratification by BRFSS region and percent of students eligible for the free/reduced school lunch program, 26 schools were randomly selected and 25 agreed to participate. Twelve examiners completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 2809 eligible students in kindergarten and 3rd grade,1276 were screened; for a response rate of 45%. In the state, 49% of public elementary school students participate in the FRL program; 44% of the students in the participating schools were eligible for FRL. The data were self-weighting and the estimates presented are not adjusted for nonresponse.

Colorado

The survey was completed during the 2010-2011 and 2011-2012 school years. The sampling frame consisted of all public elementary schools with at least 10 children in 3rd grade. Using probability proportional to size sampling along with implicit stratification by percent of children eligible for the free and/or reduced price meal program, 60 elementary schools were selected of which 58 participated. If a school refused to participate, a replacement school within the same sampling strata was selected. If the sample school plus three replacement schools refused to participate, no data were collected in that sampling stratum (n=2). Trained dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 3858 eligible students in 3rd grade, 2908 were screened; for a response rate of 75%. In the state, 45% of public elementary school students participate in the FRL program; 43% of the students in the participating schools were eligible for FRL. The estimates presented are adjusted for the sampling scheme and non-response.

Connecticut

Head Start: The survey was completed during the 2010-2011 school year. The sampling frame consisted of all Head Start sites in CT with 3 or more children. With implicit stratification by county, a systematic sampling scheme was used to select 20 sites of which 19 agreed to participate. Trained examiners completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 1001 eligible children, 774 were screened; for a response rate of 77%. The estimates presented are not adjusted for nonresponse.
Kindergarten & Third Grade: The survey was completed during the 2010-2011 school year. The sampling frame consisted of all public elementary schools with 25 or more students in kindergarten and/or 3rd grade. With implicit stratification by region and percent of students eligible for the free/reduced price school lunch (FRL) program, a systematic sampling scheme was used to select 79 schools of which 74 agreed to participate. Trained examiners completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 10225 eligible students in kindergarten and 3rd grade, 8408 were screened; for a response rate of 82%. In the state, 34% of public elementary school students participate in the FRL program; 32% of the students in the participating schools were eligible for FRL. The estimates presented are not adjusted for nonresponse.

Georgia

The survey was completed during the 2010-2011 school year. The sampling frame consisted of all public elementary schools with 25 or more students in 3rd grade. Schools were stratified into two categories, urban and rural, and an equal number of schools were randomly selected from each stratum. Of the 80 schools randomly selected, 63 agreed to participate. Trained dentists and dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 6462 eligible students in 3rd grade, 3359 were screened; for a response rate of 52%. In the state, 61% of public elementary school students participate in the FRL program; 60% of the students screened were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

Maine

The survey was completed during the 2010-2011 school year. The sampling frame consisted of all public and quasi-public schools (private schools with at least 60% publicly funded students) with 10 or more students in kindergarten and/or third grade. A simple random sample stratified by county was used to select 33 schools; 29 agreed to participate. Trained examiners completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 2652 eligible students, 1222 were screened; for a response rate of 46%. In the state, 49% of elementary school students participate in the free and reduced price school lunch (FRL) program; 36% of the students screened were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.

Rhode Island

The survey was completed during the 2010-2011 school year. The sampling frame consisted of all public elementary schools with 3rd grade classrooms. The sampling frame was stratified by urban/non-urban status then by percent of students eligible for the free/reduced price school lunch (FRL) program. A probability sample of 107 schools was selected of which 79 participated. Trained dentists completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 4822 eligible students in 3rd grade, 3266 were screened; for a response rate of 68%. In the state, 47% of public elementary school students participate in the FRL program; 61% of students in the participating schools participate in the FRL program. The estimates presented are adjusted for the sampling scheme and nonresponse.

Utah

The survey was completed during the 2010-2011 school year. The sampling frame consisted of all public elementary schools with 20 or more students in 1st, 2nd or 3rd grade. A stratified random sample of 25 schools was selected to represent the state's health districts. All 25 schools agreed to participate. A trained dental hygienist completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the eligible students in 3rd grade, 937 were screened; for a response rate of 54%. In the state, 42% of public elementary school students participate in the FRL program; 46% of students in the participating schools participate in the FRL program. The estimates presented are adjusted for the sampling scheme and nonresponse.

West Virginia

The survey was completed during the 2010-2011 school year. The sampling frame consisted of all public elementary schools with kindergarten and/or 3rd grade. The sampling frame was stratified by region with schools randomly selected within each region. Twenty-four schools were selected. Trained dental hygienists completed the screenings using diagnostic criteria comparable to ASTDD's 1999 Basic Screening Survey. Of the 2609 eligible students in kindergarten and 3rd grade, 1093 were screened; for a response rate of 42%. In the state, 55% of public elementary school students participate in the FRL program; 51% of the students in the participating schools were eligible for FRL. The estimates presented are adjusted for the sampling scheme.
