Community Transformation Grants Communities

In 2011, CDC gave $103 million to 61 state and local governments, tribes and territories, and nonprofit organizations in 36 states. CDC also gave nearly $4 million to 6 national networks of community-based organizations.

In 2012, CTG added the Small Communities Program. More than $70 million went to 40 neighborhoods, school districts, villages, towns, cities, and counties with fewer than 500,000 people. These grants helped smaller communities make lasting changes to reduce health gaps and expand services to prevent and manage chronic diseases. Also in 2012, CTG implemented the National Dissemination and Support Initiative. This initiative helped support, disseminate and amplify the evidence-based strategies of the CTG program in communities nationwide, including rural and frontier areas and in those areas with health disparities.

CTG Awardees

The 61 awardees received one of two types of awards: ​​​​​​​

  • CDC gave Implementation awards to 35 communities to use proven programs and strategies to improve their community’s health and wellness.
  • CDC gave Capacity-Building awards to 26 communities to build skills and systems to support community health efforts and ensure long-term success.
