Initiative to Eliminate Racial and Ethnic Disparities in Health
Racial & Ethnic Approaches to Community Health Across the U.S. (REACH U.S.)

Through REACH U.S., a national, multilevel program, CDC supports 40 grantee partners that establish community-based programs and culturally-appropriate interventions to eliminate health disparities.

This interagency agreement provides supplemental support to the National Center for Farmworker Health, Inc. to support three annual Farmworker Stream Forums.

Healthy Living in the Pacific Islands Initiative
The Healthy Living in the Pacific Islands Initiative is a collaborative project between OMHD, the Uniformed Services University for Health Science, and the University of Hawaii, College of Tropical Agriculture. The Initiative is focused on eliminating/reducing health disparity and promoting self-sufficiency among Pacific Island populations. This is accomplished through conducting population-based studies on Nutritional Assessments for Children and Young Adults living in the Pacific Islands jurisdictions.
