The map shows that concentrations of counties with the highest heart disease death rates - meaning the top quintile - are located primarily in Mississippi, Oklahoma, Louisiana, Arkansas, and Alabama.  Pockets of high-rate counties were also found in Georgia, Kentucky, Tennessee, Missouri, and Nevada.
