CDC’s Social Vulnerability Index
CDC’s Social Vulnerability Index uses U.S. census variables at tract level to help identify communities that may need support in preparing for hazards, or recovering from disaster. The interactive mapping application enables you to view and export customized maps showing SVI themes and other map layers at your location.

The United States Diabetes Surveillance System (USDSS) provides an atlas of diabetes surveillance data at the state and county levels. The online mapping tool allows users to view geographic patterns and examine trends over time across the United States and within states. The state atlas provides a query-based database that can display data across many indicators related to diabetes using maps, data heat maps, and a series of other interactive graphs. The county atlas provides maps, motion charts, and other interactive graphs to display data on diabetes and risk factors related to diabetes.
