Adult Dosing

Community Acquired Pneumonia

  • 600mg IV q12h x5-7 days

Cellulitis

  • 600mg IV q12h x5-14 days

Renal Dosing

  • CrCl 30-50: 400mg q12h; CrCl 15-30: 300mg q12h; ESRD/HD: 200mg q12h

Hepatic Dosing

  • No adjustment

  • Currently no pediatric dosing for this drug.

  • Allergy to class/drug (see also Cephalosporin Cross-reactivity)
  • Caution if hypersensitive to beta-lactams
