Matthew D. Ritchey, PT, DPT, OCS, MPH

Senior Scientist, Epidemiology and Surveillance Branch, Division for Heart Disease and Stroke Prevention, National Center for Chronic Disease Prevention and Health Promotion, CDC

Matthew D. Ritchey is a senior scientist in the Epidemiology and Surveillance Branch of the Division for Heart Disease and Stroke Prevention, National Center for Chronic Disease Prevention and Health Promotion, CDC. In his current role, Dr. Ritchey conducts health services research, serves as the lead epidemiologist supporting the Million Hearts® initiative, and assists with ongoing cardiovascular disease surveillance efforts. Previously, he served as the CDC Epidemic Intelligence Service officer and chronic disease epidemiology advisor for the Indiana State Department of Health.

Lawrence Garber, MD

Medical Director for Informatics, Reliant Medical Group

Lawrence Garber is a practicing internist, the medical director for informatics, and associate medical director for research at Reliant Medical Group, a multispecialty practice. He is a board-certified clinical informaticist and has had decades of experience and success in health information technology.

Dr. Garber is chair of the Massachusetts eHealth Collaborative’s Executive Committee, a prior member of the Massachusetts State Health Information Technology Council, and a former member of privacy, security, and interoperability workgroups under the HHS Office of the National Coordinator for Health Information Technology (ONC). As a proponent of “Hassle-Free HIE,” he has been principal investigator on federal and state grants to develop innovative health information exchanges, and a participant in a project that studies exchanging standard documents between Europe and the United States. Dr. Garber also co-chaired an ONC workgroup, which used an evidence-based approach to update standards for exchanging clinical information to better meet the needs of care transitions and planning.

Jessicca Moore, MSN, FNP

Associate Clinical Director, Petaluma Health Center

Jessicca Moore is associate clinical director at Petaluma Health Center in Petaluma, California. She received her master’s degree in nursing from UCLA and a certificate in rural and community management from Sonoma State. She also completed the Institute for Healthcare Improvement Primary Care Practice Coach program and is both a clinician and an improvement and innovation coach at Petaluma Health Center.
