To request the OSTLTS Director as a speaker at an event or meeting, please complete this form [PDF – 27KB] and submit it to JWalker4@cdc.gov.

A member of OSTLTS will contact you as soon as possible to let you know the director’s availability. If the director is able to participate, you may be asked to provide more detailed information. Your request will be processed within 5–7 business days of receipt.
