Annual Hospital Interview

The National Hospital Care Survey is inviting all sampled hospitals (both participating and non-participating) to complete a brief set of questions on their hospital characteristics.  This information will be used to assign appropriate statistical weights and inform non-response estimates for the survey.

• If you do not currently participate in the survey, you will be asked to complete 5 questions on basic hospital characteristics and visit counts by department for the 2016 calendar year.

• For participating hospitals, the questionnaire includes topics that cover your hospital’s general characteristics (e.g., primary service type or service changes), utilization statistics, and additional detail on how your hospital reports its data to other entities.

To complete the Annual Hospital Interview, please access the online portal at the link below; use the username and password provided previously.  The first time you log in, you will be prompted to change the password.
