Submit a Nominee

HPV vaccine is cancer prevention champion. Submit a nominee.

Nomination Process

Submit your nominations directly to CDC, by clicking the “Submit A Nominee” button on this page. You can nominate yourself or another provider.  Nominations will be accepted from all 50 U.S. states, eight U.S. Territories and Freely Associated States, and the District of Columbia. The nominee must operate within a single HHS region.

Nomination Deadline

Nomination packets must be sent to PreteenVaccines@cdc.gov on or before September 15, 2017

You must include the following in your nomination submission:

  • Completed nomination form
  • Image or photograph of the nominee, which may be used for promotional purposes

Once CDC receives all submissions, a review panel will evaluate the nominations. The panel will select up to one awardee from each of the 10 HHS regions based off the listed criteria and considerations.

Award Presentation and Recognition

Champions will be announced in October 2017. They will be featured on CDC’s website and in the #PreteenVaxNews e-newsletter, will receive an HPV Vaccine Is Cancer Prevention Champion plaque, and may be recognized at local or national events led by cancer or pediatric health organizations. Champions will also receive a digital web badge for their websites and materials to promote their success.
