mPINC Questionnaires

Every other year, every facility in the U.S. that routinely provides maternity care services is invited to participate in CDC’s mPINC Survey. At facilities that choose to participate in the survey, an individual submits the survey on behalf of his or her institution in their capacity as the person most knowledgeable about the relevant practices.

PDF documents available for download here are for informational purposes only. All mPINC Survey data collection is currently managed by Battelle Health and Analytics, a national survey and research organization with extensive experience in collection of health data.
