Administration

Adult Dosing

  • Edema: 100-150 mg PO BID
  • Hypertension: 50-100 mg PO daily or in 2 divided doses

Pediatric Dosing

Safety/efficacy not established

  • 0.5-2 mg/kg PO BID, max daily dose 300mg

Special Populations

  • Pregnancy Rating: C
  • Lactation risk: Infant risk cannot be ruled out
  • Renal dosing:
    • GFR >10: use 12h dosing interval
    • GFR <110 or creatinine >2.5: use not recommended
  • Hepatic dosing: contraindicated in severe liver impairment

Contraindications

  • Allergy to class/drug
  • Anuria, severe renal dysfunction
  • Hyperkalemia
  • Concomitant use of antikaliuretic agents, potassium supplementation/salt substitutes, or other potassium-sparing agents
  • Severe liver disease

Adverse Reactions

Serious

  • Hyperkalemia
  • Azotemia, nephrotoxicity
  • Jaundice
  • Megaloblastic anemia
  • Thrombocytopenia, purpura
  • Anaphylaxis

Common

  • Hyperuricemia, electrolyte abnormalities

Pharmacology

  • Half-life: 1.5-6.5h
  • Metabolism: Hepatic
  • Excretion: <50% renal

Mechanism of Action

  • Potassium-sparing diuretic: inhibits epithelial sodium channel in distal renal tubule
