Adult Dosing

General

For Pain, severe chronic: Dose: Individualize dose PO q12h; start 10mg PO q12h, may increase by 10mg PO q12h q3-7 days. Titrate slowly in elderly patients.

Pediatric Dosing

Currently not recommended

Special Populations

Contraindications

  • Allergy to class/drug
  • labor and delivery
  • GI obstruction
  • paralytic ileus
  • acute asthma exacerbation

Adverse Reactions

Serious

  • respiratory depression
  • apnea
  • hypotension, severe
  • paralytic ileus
  • bradycardia
  • anaphylaxis

Common

  • Constipation
  • nausea
  • fatigue
  • vomiting
  • xerostomia
  • headache
  • URI
  • tremor

Pharmacology

  • Half-life: 8 hour
  • Metabolism: Liver
  • Excretion: urine
  • Mechanism of Action: bind to various opioid receptors
