Colorado

	ColoradoColorado is implementing the use of clinical care systems that have proven successful in blood pressure control and tracking clients with uncontrolled hypertension.

Michigan

	MichiganThe Michigan WISEWOMAN program uses community navigators to help women with uncontrolled high blood pressure find needed medical services and lifestyle programs. To help strengthen the relationship between its community and health system, the navigators serve as a link between the clinic, participants, and various community resources.

Nebraska

	NebraskaMeeting women where they live and work, the Nebraska WISEWOMAN program uses community health workers to identify eligible women, determine their heart disease risk, and refer them to needed medical services and lifestyle programs.

Pennsylvania

	PennsylvaniaThrough its partnership with the state’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), the Pennsylvania WISEWOMAN program offers Supplemental Nutrition Assistance Program (SNAP) benefits to its participants. After an initial screening, women are referred to local resources for healthy eating and blood pressure monitoring.
