Healthcare professionals may also be asked to help NIS-Teen provide accurate information about adolescent vaccinations. It is very important to continue to fill out the Teen Immunization History Questionnaire (IHQ) for CDC to track accurate vaccination coverage at national, state, and selected local levels.
