Each fact sheet addresses frequently-asked questions, including what vaccines are needed for preteens and teens, when they are needed, and why.
Note: The Vaccine Information Sheet (VIS) is federally mandated to be given prior to the administration of a vaccine. The fact sheets are provided as supplementary materials and can be used in addition to the VIS but not instead of the VIS. A VIS still must be given when a vaccine is administered.
There are 2 versions of the fact sheet. The "Basic" fact sheets offer vaccine information in a question and answer format and are available in Spanish as well as English. The "In-Depth" fact sheets use a narrative style and some include a true story about a family affected by a vaccine-preventable disease.

*Each basic fact sheet is written at a 6th to 8th grade reading level, in a Q&A format; each in-depth fact sheet is written at a 10th to 12th grade reading level, using a narrative style.
About These Materials
These materials, collectively called Provider Resources for Vaccine Conversations with Parents, have been developed by CDC, the American Academy of Family Physicians (AAFP), and the American Academy of Pediatrics (AAP).
