Question 1: The guidelines for both gonorrhea and chlamydia have alternate therapies for children who weigh less than 45 kg. Do these apply to adolescents who weigh less than 45 kg., or do these individuals receive the adult dose? Also, what is the age at which to use adolescent recommendations versus child recommendations?

Answer: The FDA recommends defining adolescents as age range 12-21 years. Adult and adolescent STD treatment guidelines can therefore be applied to all patients 12 years of age or older. In any event, for chlamydial infections, the guidelines do state that children aged >= 8 years can receive adult dosing (2010 STD Treatment Guidelines, page 49).

Question 2: What is the recommendation for prepubescent HPV condyloma?

Answer: Within the STD Treatment Guidelines, there are no specific guidelines for treatment of prepubescent condyloma. The safety and efficacy of all 3 patient-applied recommended regimens for external genital warts are not established in pediatric patients (none of the 3 patient-applied regimens have FDA-approved pediatric indications). Referral to a provider experienced with provider-administered therapies for pediatric patients is advised.
