Intervention Mapping as a Guide for the Development of a Diabetes Peer Support Intervention in Rural Alabama
This figure depicts the role of peer advisors in diabetes management using the chronic care model. The peer advisor is placed centrally in the figure to emphasize her role, which is to serve as a link between the patient and the health care team. By providing assistance and support, the peer advisor facilitates productive interactions between the patient and the health care team, thereby changing behaviors and improving diabetes outcomes.
