Tailoring Care to Vulnerable Populations by Incorporating Social Determinants of Health: the Veterans Health Administration’s “Homeless Patient Aligned Care Team” Program

A flow chart describes the continuum and process of 1) treatment engagement of disengaged, high-risk, high-need homeless veterans, 2) stabilization of their housing and health care needs while enrolled in the homeless patient aligned care team (H-PACT) site, and 3) disposition (ie, can they be transferred to a traditional care site or a specialty clinic or remain in the H-PACT if their care and housing needs persist?)
